The Doctor has never been identified on the show.  Time Lord records call 
him "The Doctor."  In "An Unearthly Child" when Ian asks for Dr. Foreman,
the Doctor looks whimsical and says that he wonders where he heard that
before.  In "Spearhead from Space" the Doctor tells the Brigadier to
list him in their files as Dr. John Smith, and in "The Armageddon Factor,"
Drax calls the Doctor "Theta Sigma", which he shortens to "Thet"
