Unfortunately I don't have a reference at hand; I have the information from my friend the pediatric psychiatrist. You might try asking your pediatrician (if you think that's reasonable) or looking in a good library for a reference work on pediatrics. Failing that, just as a guess you might try T. Berry Brazelton's books.
