SYSUAF format:

The file is automatically converted to VMS V4.0 format as part of the
upgrade.   If you don't do the upgrade (i.e. build V4.0 from scratch
for some reason and then copy your SYSUAF from some other disk) then
you should search through the upgrade stuff and find the program that does
it.  I never looked for it but clearly it's got to be there.
