Cheating they're asses off. Unfortunately, "know the subject cold" != "had the class before". It is quite possible to know everything the course is going to teach, but not to have formal course credit for this knowledge. It is also possible to find the particular course so easy that reading the textbook the night before each exam suffices to get a good mark. I have seen both of these phenomena, in cases where I'm quite sure there was no cheating.
