In space seed when Kirk et al are reviving Khan, the phasor does indeed fall off Kirk's belt. Shatner doesn't seem too interested until the end of the scene, but Kelley is beside himslef with worry about it. He takes at least 4 glances down at the phasor but none off-stage. When he's supposed to squat down to look at Khan, he pushes the phasor aside and then moves into his correct position kneeling on one knee looking at Khan. At this time Shatner takes a long look over to stage right and has this do-we-cut- this-scene-or-what look on his face and then turns to Kelley with the same look. If someone cracked up this would have made the blooper reel! This looks more like a mistake than an inconsistancy because we do not see the phasor back on Kirk's belt in this scene.
