Births are usually performed with the mother on the delivery table in a horizontal position. This doesn't allow gravity to help much at all. (As a side note, doctors are [finally!] beginning to realize that if they position the mother somewhat more vertically, gravity does help as labor is somewhat less difficult and shorter.)
