(easy enough for Frith) The first thing that pops to mind is Edith Keeler in "City on the Edge of Forever". I don't think that there are any more, but there was another episode where a member of the Enterprise crew was killed by a 'primitive' transportation device. There was that lady leiutenant in "The Paradise Syndrome" that was gunned down by that airplane. This brings about another trivia question: What kind of plane was it?
