Summary: 

Having specifically looked at tapes of the credits for Logopolis, both Tom
and Peter were listed as "Doctor Who", as were all doctors up to this time.
Peter has the distinction of being the only actor credited both ways over
time (probable exception to this in 5 and 2 doctors stories).
William Harnell will forever have been listed only as Dr. Who since he died
before the change in credit listing.
