     This is a short survey for smokers.  I am posting it simply
because I am curious. No flames either way on smoking.  Please(!)
mail responses directly to me, I will summarize to individuals if
requested, or to the net if enough interest is shown.  All infor-
mation will be held confidential.  I am limiting distribution to
North America, if you feel it should be otherwise, please tell me.

     If you are a smoker or former smoker, please reply.

		     SURVEY FOR SMOKERS/FORMER SMOKERS


	1)  When did you begin smoking?

	2)  Why did you begin?

	3)  What do you smoke (cigarettes, cigars, pipe, etc.)?

	4)  Have you tried to quit?

		(If yes:)
		a) Have you tried more than once?

		b) Were you ever successful for a reasonable length
		   (in your opinion) of time?

		c) Have you been completely successful (i.e.) not started
		   again again since you quit)? How long has it been?

	5)  If you still smoke:  Why do you continue smoking with all of the
	    overwhelming evidence that this is a life threatening habit?
