1) I think Spock falls in love in "This Side of Paradise".

2) It seems to me that the Enterprise went fastr then warp 6 in
   most of the episodes or at least 50% of them, but two name
   come off the top of my head are "Is There In Truth No Beauty",
   and "The Arena".

3) I sure there are ar least four episode in which the Enterprise has 
   only a skeletal crew, however, I can only remember three names.
   
  They are:  "Day of the Dove"
             "By Any Other Name"
             "Ultimate Computer"   (I think I miss spelled this)

  The last one is when everyone comes down with a sickness and
  Kirk, McCoy, Spock go down to a planet to "make" the cure.
  On the planet is an Earth man who has lived since early Earth's
  history (I don't remember the time period, nor the man's name).
  This man has built a woman and Kirk taught her to "feel" love.
  In this episode it only says that the enterprise is down to a 
  skeletal crew, it doesn't show it.

As for the Bonus question I have drawn a blank.....
