I know I don't think those type of plans are worth the cost (from the consumer's view) for smaller appliances. However, I'm not so sure on something as expensive as a car powertrain. In that case, it might be more like collision insurance - on the average it makes money for the offering company but the very high cost of a claim (new engine, for example) may make it look attractive.
