Well, my favorite is from "The Changeling":

Kirk (to Nomad in space): We are prepared to beam you aboard.

Scotty: Captain, y'forgot about the atmosphere and all that.  Are y'sure
y'wanna do that?

I like it because of the way Doohan stays completely in character, ad
libbing the blooper.

My second favorite is from "Bread and Circuses", when the guards come
running down the hall by the jail cells, and several slip when they try
to turn a corner.
