Did Khan have the necklace before or after "hijacked" the Reliant?  If it were
after, perhaps he stole the insignia from the captain or someone else on the
ship and fashioned it into a necklace.

I don't remember if he had the necklace while he was marooned on Ceti Alpha 5.
