Aviation Leak is reported as saying:
"...  The SRB blew a seal.  The resulting jet severed the
lower connection to the main tank...  The SRB pivoted and
hit the top of the main tank, breaking the internal tanks
and causing the explosion..."

Aviation Leak also claims that this is NASA's opinion.
