I was glad to see your request for info, as I have been trying to locate some of that type humidifier for a couple of years and all the store clerks look at me as if I'm crazy when I describe them. I had begun to think I had only imagined having seen them before. If you get any responses on where available I would appreciate the info. I did locate something similar around here. It was a small, thin, galvanized, sheet metal container designed to slide into the small openings in the middle of the elements of the radiator ( that is it fits through the middle of the radiator ). It has a small flared neck that is left extending from the end of the radiator for filling. I did not buy one because (1) it seemed too expensive (around $10 I think), (2) the water capacity seemed very small and so it would need constant filling, and (3) I didn't think I would like the look of the galvanized metal neck extending from the radiator. The store I found that in was Dick's Direct Sales in Hope, NJ (It's a regular hardware and building supplies store, not a large discount house as the name might imply).
