At least on the high school and college levels much of the use of steroids, painkillers, etc. has been alleged to come from the coaches supplying the drugs and pressuring the atheletes to use them. I have heard some the same on the pro level (clearly excepting the use of "recreational" drugs.) --henry schaffer
