I was very disappointed to find that LaTeX doesn't have full index
support, but instead leaves you with a .idx file that needs a great
deal of additional processing.  Does anyone have a program that takes
a LaTeX index file and turns it into a useful index (with \item and
\subitem lines, and all that)?
