This is poorly documented in both Microsoft's manual and an allegedly
helpful book on the subject (published by Microsoft Press, which may be
the problem).

I swore at this one for a year after buying my MS-Word.  Finally, I found
the trick--make sure that the location of the running head is within
the page heading.  I don't have the stuff in front of me, but the heading
must be less inches from the top than the top margin is high.  If in
doubt, exaggerate (.5" and 2", respectively) until you get it where
you want it.
