I haven't received much information so far, but following is a summary of the responses I did receive: [From Larry Tesler] A little information is available in the new book "Object-Oriented Programming for the Macintosh" by Kurt Schmucker, Hayden Books, 1986. [From Mike (mike@bellcore.com)] Try looking at how browsers are constructed in the third (red) book. Also try looking at the MouseMenuController -- this is the most basic controller, and the one on which everything else hinges. Also, several people pointed out that Tektronix' "Advanced Course" on Smalltalk-80 covers the Model-View-Controller paradigm, along with other things.
