Copies were distributed with MDS at one time.  The phone book version of
IM is out of date and has been superceded by the hard cover bound 3 volume
set or the soft cover individually bound 3 volume set.

There is now a new volume of IM (Volume IV) which covers the Mac+, HFS, etc.
