I have had this occur when a software contractor redefined the logical
name for where temporary mailboxes are supposed to be defined in the
system logical name table to a logical name table which users had no
privilege to write in.

Check to see where the logical name LNM$TEMPORARY_MAILBOX points to.
This logical name usually points to the LNM$JOB logical name table but
can be redefined.  If it points to the wrong place the job cannot create
a temporary mailbox and will not spawn.
