That's probably NOT a hardware. There is a bug in ProDos 1.1.1 that will trash track 0 (the catalog track) whenever the moon is full. It has something to with the new code that supports 3.5 inch disks. A fix was published in Open Apple a while back. Until you get the fix, don't use version 1.1.1.
