Two that I know of are the IBM PC-DOS Technical Manual which includes the source code for ramdisk program, and "Advanced MS-DOS" by (I think) Ray Duncan and Published by Microsoft Press. The latter book is truly excellent and goes into considerable detai writing and using DOS device drivers.
