One thing that I've found with the modular plugs is that over time, the contacts get bent so that they don't have a solid connection. The simple solution (which may or may not solve your problem) is to take a small screwdriver, paperclip, or swiss army knife (No, of course we don't hack...) and carefully bend the contacts out.
