It seems to be a screw-up in the handshaking between the various boxes and the SCC running the serial port. In the case of the MacVision, it will either flag a "no signal" error or it will sit around sweeping (vertically) only one of the collumns of video input. Since neither of these units have an equivalent to a "hardware reset" button, I am usually forced to power down everything in order to get it working again.
