The suggestions came in basically two categories:
1) Use Glue; and 2) Distribute the PostScript source.

Glue works very well, but for this application I'm not really
interested in licensing the ImageViewer application to
distribute with the document.  I neglected to mention that my
document was in landscape mode, but it turns out that the
Glue-generated MacPaint image can't be read into MacPaint (or
SuperPaint) properly.

So my solution is to distribute:
1) the original Word document, in case the recipient has
access to a system with Word; and
2) the postscript file, generated from Word with Command-F,
and the (free) SendPS application which can be used if the
recipient has access to a LaserWriter.
