Vulcan Binder & Cover, P.O. Box 29, Vincent AL  35178 (800) 633-4526 has them.
AC09100 is a 1" binder (200pp) with a clear overlay on the front and back to
hold custom markings, spine too.  They are $3.63 each, minimum of 12.

AB09116 (white) AB09216 (black) is the same without the overlays, $2.48 each,
minimum of 12, add $5.00 for broken carton.

A410-06 is a vinyl D ring binder with slip case both with clear overlay,
$7.50 each qty 12.

Linen finish cloth sets are available (price includes 4 line imprint), binder
alone FX10906 $3.80, slip case FY10900 $4.20, both FZ10906 $7.50, qty 12 min.

I bought a dozen from them and split the order with some neighbor sites.  That
was my only dealing with them, pleasant enough.  There's another outfit,
Anthropomorphic Systems Ltd, 376-B East St Charles Rd, Lombard IL 60148
(800) 332-5669 (312) 629-5160 that sells the binders and slip cases in colors,
imprinted or not.  They also have continuous form paper for the small manual
size.  I have never dealt with them.
