You might try defining a special character or two in your nroff tab
file.  We've used this successfully with our LaserJet to turn on and
off underlining; we use:

	US 0 \033&dD
	UE 0 \033&d@

to start and stop underlining, so that we can have regular, bold, and
true italic; with any of the three underlined as well as normal.
