Summary: BPB->flags - what are they?

A few questions about TOS (and MS-DOS) disks:

In the BIOS parameter block, the 'flags' field: what are the flags?

Where is the disk ID number stored?

Where is the info on whether the FATs are 12 or 16-bit type stored?

Please reply directly, I'll post including some other info.
