I've seen the output of MDPIC, and it's not bad. For anything that pic supports directly (arcs, boxes, circles, ellipsed, lines, arrows, etc.) MDPIC works fine. It also works well with things like rounded boxes, which are easily handled by pic. It's not so great with shading, as it has to simulate that with dots or dashed lines.
