You don't need an init, just a copy of the icon (no, I don't
have one) which you then paste into the finder using resedit.  
You could always make an icon using any of the pd or shareware icon tools.
Most people are good enough at drawing to
copy something as small as an icon with reasonable accuracy.  (Needless
to say, try it on a backup finder copy and not on your original).
