I know this came up recently, but I foolishly failed to save the
response.  I need to produce dictionary-like page headings, with first
and last things from a pair of columns in the heading.  If someone
knows a solution I would be grateful for it.  All I've managed to do
through my own attempts is to reaffirm my suspicion that I know
nothing about output routines.
