While getting it ready to ship to the manufacturer he spotted something yellow inside the drive. Upon closer examination, he found a 1" x 1-1/2" post-it note stuck on the drives read/write head. Upon removing it the drive worked fine. When the owner came into the shop to pickup the drive the tech found out that the owner was using post-it notes to label his disks.
