Proof- reading doesn't catch everything, especially when you're documenting a moving target like Color QD or Styled TE (just starting to settle down now). People like us want the documentation available as soon as we can get our hands on the hardware. I would much rather have access to the 99% correct information such as IM V, knowing that when the errors that are going to slip through the cracks get caught, I can find out about them and get replacement pages to put in my reference manual. My opinion of loose-leaf vs. perfect-bound is that for something like IM that I want to keep open next to me while I'm working dictates loose-leaf. I can't keep IM IV or V open to the pages I want while I'm at the keyboard unless I either weight the pages down or break the spine -- those are not good solutions.
