Most of the responses have said that loose-leaf is better, but ... diffucult to keep manual and index updated, losing and tearing of pages, and needing larg binder(s). Someone suggested a pamphlet type system and this sounds excellent. First of all these can be simply made strong enough to not even need a binder and survive on their own, but obviously they could be put into a binder if you want. Since they will be somewhat bound you don't have the same chance of losing a single page (but then again sometimes I can't find my IM's). And the problem with updating can be made very efficient by giving each chapter its own pamphlet or 3, and if the chapter has more than one pamphlet then a cross between handwaving and a little foresight could concentrate the material that is most likely to have updates in one of the pamphlets. And of course a new index would be published with each set of updates.
