Unless you are using X Windows or Smalltalk-80, both of which are going to be more and more important to all of us. Now, when talking small, I have to use both arms to push the middle and right buttons. That is, I have to push the Command key and click the mouse to simulate pushing the right button on a three-button mouse. This is a pain. Smalltalk-80 was designed around a three-button mouse.
