The clarification (hopefully..) Specifically, the type of transition I am interested in is one where the software transfer is a one time deal. That is, group A develops the software for group B. When development is complete, the software is transitioned completely to group B. Groups A and B may be down the hall from each other or they may be separated by a great distance.
