What may have happen- ed is a poor pin connection when you removed the chip from the //c to the II+. When I first got one, switched it back and forth between my II+ and my //e and eventually the pin connection to the actual chip probably broke and it did weird things...I'd wiggle it and then it'd work but later it'd die again. I got it replaced and since then I haven't taken it out so it doesn't have any problems anymore. Their packaging (the chip) is REALLY cheap so like it says in the manual, don't take it out.
