Thanks for all of the helpful suggestions.

We now it have it working using a few fewer cylinders and sectors than the
specification sheet suggested.

# dkinfo sd1
sd1: Emulex MD21 controller at addr 140000, unit # 1
1218 cylinders 15 heads 33 sectors/track
