With Ali Ozer's help (thanks, Ali), I finally got my private icon associated
with an application.  Along the way I learned, however, that the Workspace
manager will only load icons from your private Apps directory and those
of the system (including Demos).  Here's the list:

~/Apps:/bootdisk/Apps:/bootdisk/NeXT/Apps:/bootdisk/Programming/Demos
