It is included in DECwindows  (Ultrix Window System 2.0) 
The style guide is order number AA-MG20A-TE
The number for Ultrix WS full doc kit update is QA-0JQAA-WZ2.0
 don't know what they would cost but this will give you a start with a
DEC sales rep.
