The one I have is the September 88, 2nd Draft. It was in .dvi format. However, it has been generated such that there are a lot of unresolved references, therefore a lot of ''s. There is also no index. I tried to generate .dvi from the .tar file. However, my system doesn't seem to have 'texindex'. Given all that, I would like to have a .dvi file with all the index pages and without references. Having the source to feed into TeX may cause problems.
