This happened to me once. As it turned out, I had used the Finder to make a file-by-file copy of /SYSTEM.TOOLS and /SYSTEM.DISK, and was trying to install from that. But the Installer script requires the file FINDER.DEF, so that new users have some Finder defaults. In a file-by-file copy, Finder doesn't copy it's own data files, so the copied disks wound up missing a file the script wanted to Install. Actually, this happened to me one day before 4.0 was supposed to go Golden Master, on a Saturday. Caused some of the Apple II System Software people to get worried enough to come in on Saturday morning to track it down. You should have heard the combination relief/annoyance when they found it wasn't a GS/OS or Installer bug.
