Use the old system 4.0 icons.  Also, see my previous posting on
generating postscript & printing it out.

This should fix your problem.

{appledict #68

is a kiss of death.  This appledict uses postscript commands that are
too new.  I believe you want {appledict #47  or something like that.
I solved the same problem on our imagens (2 kinds) by using
the older icons (ONLY these icons work, don't ask me why).
