I ran into this while debugging some code the other day.  It turns out that I
was attempting to delete a node that was pointed to by a pointer which was off
in never-never land.  The OS error made it impossible to execute a trace-back
and made the bug very hard to find.

Of course, fixing my buggy code made it go away :-)

I'm also interresting in what happened here.
