Thanks to INMOS and Adrian for their prompt replies - all is *now* clear.

Using [EXTRACT] creates the .CSC file which contains folds. The book
didn't mention them so I didn't look for them.

If instead I had used [WRITE HOST] on the CODE SC fold the resulting
.XSC file would be as specified in the book. It works.
