Ok, lets try again.

RES0: is for PREPing the first ST506 drive
RES2: is for PREPing the first SCSI drive.

The sequence is


mount res0:
prep res0:

(answer the questions properly; if you enter incorrect information
 the drive may not be found)

REBOOT THE MACHINE

Now, drive dh0: should be automounted.

do an 
assign

and an info

and see what drives you see in the lists.  you should see
a DH0:

Now format dh0:

That's pretty much it.
