To get the DORVAK keyboard layout on an Apple //e requires a small bit of motherboard hacking. The Apple //e engineers put DORVAK capabilities into their machine, but only the Apple //c and Apple //gs engineers made DORVAK easy to get at. I've got a copy of CALL - A.P.P.L.E. which has the instuctions for the mod.
