Cost about $.05. It is much more common for polarity to reverse on any _completed_ (i.e. answered) call. The fact that you do not get reversal on, e.g. (312)555-1212 or "your call cannot be completed as dialed" is accomplished by _not_ indicating "off hook", while allowing conversation.
