Have NASA ever managed to run cleanly through the countdown sequence and
launch a shuttle mission at the first attempt? The Magellan mission nearly
qualified, but ran into trouble about 1hr:30 before launch, and finally aborted
at T - 31sec.

I don't remember *any* clean launches, but then again, we get virtually no
media coverage over here :-(
